Respite Service
Do you have regular care from a live in Carer, who is taking a break or has personal issues to attend to? CQ Community & In-Home Care can provide a respite service to allow your Carer the time away from you, worry free. Respite can be provided based on tasks or time, that is, we can provide a Care Support Worker to assist you with Activities of Daily Living such as having a shower, taking your medication and monitoring your blood sugar level; or a 24 hour care supported service… choice is yours.
You live at home, but have had an illness or injury that has landed you in hospital? Transition Care supports you to get back on your feet through intensive options initially, tapering off as your health improves. CQ Community & In-Home Care Support workers offer you various approaches to maximize your health outcomes. Some Transition Care options may be financially supported through funding via the Queensland Health Transition Care Funding Model.
Pre & Post Hospital Care
Do you require surgical ‘prepping’ or after care? CQ Community & In-Home Care can provide experienced personnel to assist you to be fully supported prior to admission for a surgical service, and again on your return from hospital / Day Surgery. Often you are well enough to be discharged to the safety and comfort of your own home, however, without the capacity to be discharged into the care of an adult once you have been administered an anesthetic agent, you are often required to remain in hospital overnight, both increasing your care costs and decreasing your comfort – we all know – nothing beats your own bed! This is a well regarded service by both medical and hospital personnel – why not ask us what it entails?
In-Home Care
Are you finding it increasingly difficult to manage at home without some support? CQ Community & In-Home Care can offer a range of supports which will keep you as independent and well as your health will allow, within your own familiar environment.
These supports may include but not be limited to:
Assistance with ADL’s (activities of daily living – showering, dressing, eating etc);
Assistance with / supervision of medications;
Wound care management;
Diabetes management;
Assistance to attend appointments
Assistance with maintenance of independent activities such as shopping, gardening, meeting friends & family etc;
Care from 1 hr to 24 hrs / day, 7 days / week, 52 weeks / year, and anything in between;
Transport Assist
Do you need assistance to travel away to meet family or medical commitments, but feel too vulnerable to travel? CQ Community & In-Home Care can provide staff who are experienced at getting you safely to where you need to go and back again…
Palliative Care
Do you have special needs associated with an incurable medical condition which is impacting more and more on you and your family?
